At Liv'n With a Purpose, we understand the significant impact that mental health conditions can have on an individual's ability to perform daily activities, including work responsibilities. In alignment with the Family and Medical Leave Act (FMLA), we provide documentation for clients who require leave due to a serious health condition, including mental health conditions.
Criteria for FMLA Letter Eligibility:
- Current Client Status: FMLA letters will only be provided to individuals who are current clients of Liv'n With a Purpose.
- Minimum Duration of Therapy: Clients must have been actively engaged in therapy with Liv'n With a Purpose for a minimum of three months, with weekly sessions without interruptions.
- Frequency of Sessions: Clients should have attended therapy sessions regularly, with a frequency of at least 1-2 sessions per week during the six-month period, without interruptions.
- Demonstrated Need for Leave: During therapy sessions, clients must have demonstrated how their mental health condition significantly impacts their ability to work or perform daily activities, thus requiring FMLA leave.
- Clinician Involvement: Your therapist must observe and document how your mental health condition affects your functioning and may support the need for medical leave under FMLA.
Process for Obtaining an FMLA Letter:
- Initial Assessment: Clients who believe they may need medical leave due to a mental health condition are encouraged to discuss their situation with their therapist during their sessions.
- Assessment Period: Following the initial discussion, clients will be observed and assessed during subsequent therapy sessions to evaluate the impact of their mental health condition on their work and daily activities.
- Documentation: If, after the assessment period, it is determined that a mental health condition requires medical leave, the therapist will document their observations and provide a recommendation in the client's file.
- Issuance of FMLA Letter: Upon meeting the eligibility criteria and receiving a recommendation from their therapist, clients may request an FMLA letter from Liv'n With a Purpose.
Important Note:
Liv’n With A Purpose reserves the right to decline or revoke an FMLA letter if we believe it is not being requested or used appropriately. If an FMLA letter is revoked, the FMLA program will be notified.
FMLA letters provided by Liv'n With a Purpose are intended solely for the purpose of supporting clients in taking medical leave due to a serious health condition as outlined by the Family and Medical Leave Act. These letters do not grant individuals the same rights or privileges as those with disabilities covered under the Americans with Disabilities Act (ADA).
INSURANCE EAP PLANS: EAP services cannot be used to access or start documentation for Emotional Support Animals (ESA), FMLA, Short Term Disability (STD) or Long Term Disability (LTD) eligibility determination.
